We understand it can be an emotional process to transition a loved one to a different living environment. The Cohen Home provides support for families as we welcome new residents to our family.

We encourage you to ask questions, communicate often, participate in monthly theme-night dinners, birthday celebrations and holidays, or simply to join us for lunch or dinner when it is convenient.

Moving In

To get started, The Cohen Home eases the transition by learning about your goals and care needs first, then customizing a plan so you feel welcome, comfortable, included, and engaged from the first day you join our family.

Caregiver Support Group

For extra help, we invite you to attend our caregiver support group held on the second and fourth Tuesday of each month, 6:30 – 7:30 p.m. It is free and open to the community. Click here for more information.
