ASSISTED LIVING

At The Cohen Home our residents have options for what they would like to do for themselves and what they would like our staff to do for them. Coordinated by Registered Nurses as well as 24-hour onsite, trained caregivers, residents receive assistance with:

  • Activities of daily living (ADLs) such as dressing, hygiene and grooming
  • Transportation to group activities and individual medical appointments
  • Medication management and reminders
  • Weekly housekeeping and laundry
  • Participation in activities that spark engagement and socialization
  • Special diet management
  • Regular wellness checks by our nursing staff

Individualized Plan of Care

Before moving in, we work with you to create an Individualized Plan of Care that works for both you and your loved one. Each resident is evaluated by our experienced staff, preferably in his or her current living environment, to get a true picture of the care needs and to discuss your goals and expectations. We also re-evaluate residents periodically to anticipate changing care needs and work in partnership with the resident and family to adjust the care plan accordingly.

Personalized Care Levels

At The Cohen Home we encourage our residents to live as independently as possible, always aware that our compassionate assistance is waiting in the wings. Our three progressive levels of care allow us to personalize and accommodate our residents’ needs while maintaining familiar friends, caregivers and surroundings.

Level I: Basic Assistance

This level is for residents who are independent in many areas of hygiene and activities of daily living yet requires some assistance with services such as cooking, cleaning or laundry.

Level II: Moderate Assistance

In addition to basic assistance, this level is for residents requiring a greater level of support with escorts, activity or mealtime reminders, hygiene such as bathing and dressing, and other activities of daily living.

Level III: Maximum Assistance

In addition to all of the above, this level is for residents who require more frequent and intensive support or assistance with escorts, activities, dining, hygiene and activities of daily living.

Dementia Care Neighborhood

The Extra Care Neighborhood at The Cohen Home is for residents who need our maximum level of care and supervision due to dementia or significant physical impairments. Residents in our extra care neighborhood require frequent and intensive assistance and receive all the services and amenities outlined in the Assisted Living Levels I, II and III.
